SKILLED NURSING
Our full-time licensed RNs and LPNs are staffed for clients who need a higher level of healthcare management and treatment. Our skilled nursing component complements our non-medical care services and helps ensure a complete continuum of care for our clients. Skilled nursing services are perfect for patients who are recuperating after a hospital discharge and seniors who are suffering from chronic illnesses.
medical social worker
Our goal is to help patients achieve and maintain their maximum level of independence and wellbeing. By providing the appropriate education and skilled services, patients are able to safely remain in their homes once home health is no longer required. ***Social Workers are available in conjunction with medical necessity services such as physical therapy and or nursing not as a stand alone service.
home care aides
Our state-certified home care aides (Certified Nursing Assistants and Home Health Aides) are trained to provide personal care and home management to our clients. They assist patients with all daily living activities. Our home care aides are carefully selected in order to safeguard your health.
medical supplies
Medical supplies are items that, due to their therapeutic or diagnostic characteristics, are essential in enabling home health agency personnel to conduct home visits or to carry out effectively the care the physician has ordered for the treatment or diagnosis of the patient's illness or injuryWe offer patients medical supplies at no cost as is reasonable and necessary for patient’s care and as ordered by the physician.
physical, occupational, speech and therapy
When you’re recovering from an injury, illness or surgery, the process can sometimes mean learning how to complete daily tasks again. Our physical, occupational and speech therapists work with you to help you regain mobility through a wide range of rehabilitation services.
We deliver services according to a plan of treatment developed by patients, their family members, physicians and our home health staff to maximize independent functioning. Our skilled professionals can help you if are recuperating from surgery or with the management of a chronic condition.
durable medical equipment
We can arrange durable medical equipment through an outside supplier. Patients bill directly for these services.
