Disease Management

With the prevalence of chronic diseases growing, management programs are playing a significant role in an attempt to improve patient outcomes. It is estimated that chronic diseases, such as diabetes mellitus, congestive heart failure (CHF) and hypertension, account for up to 75% of total annual health expenditures. Disease management programs offer self-management education which may include prevention, behavior modification and monitoring of compliance to obtain the outcomes specified by the physician and avoid rehospitalization.

Southeast Louisiana Home Health has developed various disease management programs to help guide the care of the patient suffering from a chronic disease by offering a system of coordinated healthcare interventions. Our clinical staff will work closely with the physician to keep the care plan updated so the best possible outcomes wll be achieved and rehospitalization avoided. Our programs include: Congestive Heart Failure(CHF), Chronic Obstructive Pulmonary Disease (COPD), Wound Care, Orthopedic, Mental/Behavioral Health, Diabetes and Fall Prevention.
